is your financial policy?
Payment is expected at the time of service
unless other arrangements have been made with a member of our Business Office.
Payment may be made by cash, check, money order, or we accept VISA, Mastercard, Discover and American Express.
HMO Managed Care Patients: It is your
responsibility to obtain all necessary referrals as required by your insurance
plan. Failure to do so will result in having to reschedule your appointment. We
realize the inconvenience this would cause. For that reason we want to make
it clear the importance of requesting the necessary referral.
What
insurance do you accept?
Our physicians on insurance plans through
the Genesis Physician's Group and direct contracts. We participate with Traditional Medicare
and many PPO organizations. Not all of our physicians are contracted with
the same plans, therefore it is imperative that you contact your insurance
company prior to making an appointment. We do our best to inform a
patient if we are not contracted with a particular plan, however, due to the
constant changes in employer/insurance relationships and other changes within
the insurance industry it is very difficult for our staff to know the details of
every insurance situation.
How do I get a
refill on my prescription?
We have a designated phone line for
prescription refills 214-750-4590. This RX hotline is monitored by our nursing
staff between the hours of 9:00am to 4:00pm daily. Prescription renewals called
in after 4:00pm will be handled the next business day between 9:00am and 4:00pm.
Whenever possible, prescription renewals should be called in as soon as possible
before your supply is exhausted. Information necessary for prescription refills
includes your name, date of birth, medication, date of last refill, pharmacy
phone number and your physician’s name.
What are
your office hours?
Monday through Friday from 8:30 a.m. –
4:30 p.m.
What should
I bring to my appointment?
Bring your insurance card, a photo ID and
your current medications. Some
insurance carriers require you to have a referral or primary care authorization
before treatment is given. Please check with your carrier if you are in doubt.
If you have current x-rays and/or MRI’s please bring these with you. If
possible please bring your completed registration forms which are available from
our website or we can mail them to you prior to your appointment.
